Family Case Management provides coordination of health and social services for eligible families with a pregnant woman or infant. Our goals are to reduce infant mortality by ensuring early and consistent prenatal care for women throughout their pregnancy. FCM facilitates necessary early periodic screening diagnostic testing, and required immunizations for children. Our families get help in finding an ongoing source of medical care and financial assistance. They are connected to nutrition services, health education, transportation, mental health services, substance abuse treatment and other services they may need. FCM participates in community initiatives such as county wide developmental screenings coordinated by the Regional Office of Education. FCM promotes injury prevention and child passenger safety through participation with Quad City Safe Kids Coalition. For a schedule of local car seat safety checks please click here.

Women and children come into our services through the Department of Human Services, requests for pregnancy tests, and referrals from physicians, agencies or individuals. Rock Island County has been successful in lowering our infant mortality rate. The numbers of infants and children appropriately immunized continue to increase and are well above the state average.

Illinois' universal child health insurance plan, called All Kids can be initiated at the Health Department. Our staff provides assistance in filling out the applications.

Family Case Management formerly known as Healthy Moms/Healthy Kids also coordinates an incentive program for pregnant women who live and receive prenatal care in Rock Island County. This program is called the NEST. It is funded and staffed by the Junior Board of Rock Island. Clients who participate in the program redeem their points for baby items at the Rock Island K-Mart.

Did you know?

  • In just 20 years, the percentage of children under five years of age enrolled in child care has soared from 30 percent (1970) to 70 percent (1993).
  • According to the National Center for Education Statistics, nearly 88 percent of children whose mothers work full-time and 75 percent of children whose mothers work part-time are enrolled in child care.
  • The Child Care Nurse Consultant program was established in response to the growing need for additional support services in the area of health and safety in child care.
  • Funding for the program was provided through a grant from the Illinois Department of Human Services.


Health Works of Illinois

Rock Island County Health Department (RICHD) is one of several Lead Agencies in the State of Illinois. RICHD over sees a 10 county area in assuring medical case management to children 6 years and younger who are placed in foster care. Health Works of Illinois (HWI) ensures medical needs are identified and addressed.

HWI medical case managers assist the Department of Children and Family Services caseworkers by working with foster parents, health care providers, dentists and other appropriate services in obtaining appointments and follow up.

Health Works provides in-service training for DCFS staff, physician offices, private agency staff and foster parents.

We want all kids to grow healthy and strong. All Kids Health Insurance Plan matches a plan to your income. All Kids is complete health insurance for your child. All Kids covers doctor visits, hospital stays, prescription drugs, vision care and eyeglasses. All Kids covers regular check-ups and immunization shots. All Kids also covers special services like medical equipment, speech therapy and physical therapy for children who need them.

Some families get All Kids for free. Some families have to pay monthly premiums and co-payments for All Kids Health Insurance. The amount of premiums and co-payments you will have to pay depends on your income and the size of your family.

Moms & Babies

All Kids has two programs for pregnant women: Medicaid Presumptive Eligibility offers immediate, temporary coverage for outpatient health care for pregnant women. Moms & Babies covers health care for women while they are pregnant and for 60 days after the baby is born. Moms & Babies covers both outpatient health care and inpatient hospital care, including delivery.

Hearing & Vision Screening

Hearing and Vision Screenings are provided, without charge, for all preschool children attending private or public licensed preschool and day care facilities. The Hearing and Vision program participates in collaborative preschool screenings held annually in all school districts of Rock Island County. The Health Department may be contracted to provide hearing and vision screenings in private and public school districts and special education facilities.

Lead Poisoning Prevention Program

With the Title X law in effect, the involvement of the Federal Government and the United States Environmental Protection Agency, our population of lead poisoned children in Rock Island County should decrease. The educational brochure from the U.S. EPA is to be given by landlord to all tenants. The brochure has very informative basic education on the prevention of lead poisoning in children. It is something all families should read, as lead is found everywhere in our environment.

We continue to screen children, do follow-ups, case manage children with elevated blood lead levels, complete public health nurse inspections and provide community education.

NEST

What is NEST?

The single most important factor in the birth of healthy baby is prenatal care. The NEST seeks to improve the health of pregnant women and their babies by providing:

  • an incentive for you to receive early and regular prenatal care
  • an opportunity for you to participate in educational programs to improve your understanding of health issues related to pregnancy and birth
  • information and referrals to community resources to help with special needs for you, your unborn baby, and your family.

Who should use the NEST?

  • Any pregnant women who lives and receives prenatal care in Rock Island County

The LINK is a comprehensive school linked health clinic with the ability to provide preventive medical care, acute medical care, referrals and health education in the schools as needed. Services are for children and adolescents. Click here to view current immunization recommendations.

The LINK provides on-site care 8 - 4:30 Monday through Friday and 24 hour coverage is through Ilini and Trinity Emergency Rooms. The LINK can serve as the medical home for children and adolescents or can compliment services provided by existing health care providers. The LINK coordinates care with the other medical providers, social service agencies, mental health providers, and other programs and programs and organizations.

Three local pediatricians, three nurse practitioners, RN's, and a helpful office staff provide services at the LINK. Our mission is to provide medical care to all school age children so that they go to school ready to learn physically, emotionally, and mentally.

WIC provides food coupons for pregnant, breastfeeding and postpartum women. WIC strongly encourages pregnant women to seek prenatal care right away. It is important to make an appointment with a doctor as soon as pregnancy is suspected as well as with your local WIC agency. Early prenatal care gives your baby the best chance to be born healthy and stay that way. After delivery, it is important to obtain proper nutrition to maintain energy and replace depleted nutrients. WIC also is a good support center for its' breastfeeding mothers. Many of the local staff are certified lactation consultants and available to answer questions. Newborns and very young infants who qualify for WIC are eligible until they are 1 year old. Newborns are measured and assessed for proper growth and feeding habits. Moms are educated on proper breastfeeding techniques and the signs of a healthy breastfed infant. The Rock Island County Health Department Breastfeeding Peer Counselor Program has been developed to provide personal assistance to new moms during the first few weeks of breast feeding.  The counselors all have breastfeeding experience and are available to answer questions and address your concerns at 309-558-2891. If formula is being used, proper amounts and preparation are discussed.  If your infant needs a specialty formula, print out the following form to take to your doctor (formula form).  Introduction of solid foods throughout the first year are presented to the client by video education, group education in a workshop format or individual counseling. WIC and the Healthy Moms/Healthy Kids program also maintain a record of the infant's immunizations when provided by the client to ensure proper vaccination. Referrals are given if there is a financial issue and parents are provided with an immunization schedule for infants and children up to 18 years of age.

Children ages 1-5 are seen at WIC and are assessed for growth and development, iron levels and eating habits. WIC also follows up on the child's immunization record and will perform a lead screen when the child is one year old. The focus of the nutrition counseling for parents of small children is to ensure proper foods are being offered and that mealtime be a pleasant, positive experience. Types of education for children are the same as listed above for infants. One of the newest forms of client education is offered over the internet and is targeted towards our low risk population. The site offers the client nutrition information and the convenience of simply printing off a certificate at home and bringing it in to receive more food coupons. The Ellyn Satter approach, which is known for its "division of responsibility" is discussed and encouraged for parents to practice. This approach has been shown to keep your child from becoming a picky eater while teaching them to eat a variety of foods and at exactly the right amounts. The Choosy Kids program was a special project aimed at increasing physical activity in children combined with nutrition education.

Women's Health Services consists of Family Planning, Female STD Clinic, Routine Physicals and the Illinois Breast and Cervical Cancer Program. These programs provide health education for healthier life styles, promote reproductive health, including pregnancy planning and STD prevention, and referral for screening and diagnostic services.
